About the event

Transitions across care settings can be particularly challenging for someone living with dementia.

Together we will look at the key principles of behaviour change and how best to support someone as they transition from one care setting to another.

We will also explore the strategies we can use to best respond to changed behaviour through the lens of real-life examples.

One of the main benefits of this workshop is the coming together of staff from different care settings across Residential and Community Aged Care and the Acute sector. By bringing the sectors together, we will navigate shared goals and perspective with the central question being: what does a good transition look like?

Given the limited places available for these workshops, please note that by submitting this form you are submitting a registration of interest only. Final confirmation of availability will be provided 1 week prior to the event. We recommend you place a holding invite in your calendar to ensure this day is kept free.
